This website is operated by Adam MDPC, trading as Human Medical Aesthetics, a physician-led aesthetic medicine practice at 629 Park Ave, New York, NY 10065 and 231 Washington Ave, Garden City, NY 11530.

When you submit the consultation form, we collect:

• Your name
• Your phone number
• Your email address
• The treatment you told us you are interested in
• Anything else you choose to write in the message field

We do not ask for, and you should not send us, detailed medical history through this website. Bring it to your consultation instead.

To contact you about the consultation you asked for, and to answer your question. That is the only reason we ask.

If you later become a patient, information you gave us here may be added to your patient record, which is governed by our practice’s Notice of Privacy Practices rather than by this policy.

Your enquiry is delivered to the practice through [ form processor — pending B2 ], and stored in [ lead destination — pending B1 ]. These providers process the information on our behalf and are not permitted to use it for their own purposes.

We do not sell your information. We do not share it with advertisers. We may disclose it if the law requires us to.

We use Google Analytics 4 to count visits and see which pages people read. It records pages viewed, approximate location from your IP address, and the site or search that sent you. It does not receive your name, email, phone number or anything you type into a form, and we do not use it to advertise to you individually. You can opt out of Google Analytics on every site you visit with Google’s browser add-on.

Our host, Cloudflare, records basic request information such as pages viewed and approximate location, in order to serve and protect the site.

We keep enquiries for two years, then delete them. Patient records are kept for the period New York requires of a medical practice, which is six years from the last entry, and longer for a minor. If you ask us to delete your enquiry and you are not a patient, we will.

You can ask us what we hold about you, ask us to correct it, or ask us to delete it. Write to doctormauer@me.com and we will respond.

Information you send through this site is transmitted over an encrypted connection. No method of transmission over the internet is completely secure, and we cannot guarantee absolute security.

This site is not directed at anyone under 18 and we do not knowingly collect information from them.

If we change this policy we will update the date at the top of this page.
